Propper Chiropractic ("we," "us," "our") runs www.propperchiro.com. This policy explains what information we collect through this website and when you call, text or email us, how we use it, and the choices you have.
Information we collect
Information you give us. When you request an appointment or contact us through our website, we ask for your name, email address, phone number and the reason for your visit. You can also give a preferred date and time and a short message. If you call, text or email us, we receive your phone number or email address and the content of your messages.
Information collected automatically. Like most websites, our hosting provider keeps standard server logs, which include your IP address, browser type, the pages you visit and the date and time of your visit. We use these logs to keep the site running and secure. We also briefly use your IP address to limit repeated form submissions; we don't save it with your request.
Analytics. We use Google Analytics to understand how visitors use our site, for example which pages are visited and how people find us. Google Analytics uses cookies and collects information such as your browser, device, approximate location and the pages you view. We use it only to improve our website, not for advertising. You can opt out with Google's browser add-on at tools.google.com/dlpage/gaoptout. [CONFIRM: GA4 is live in production; see item 2.]
Cookies. Apart from analytics, our website does not set advertising cookies, and we do not track you across other websites. Our contact page shows a Google Map, which is loaded from Google and is covered by Google's privacy policy.
How we use your information
We use the information you give us to:
- respond to your appointment request or question and schedule your visit;
- call, email or text you about your request and your appointments, including sending you an email confirming we received your request;
- keep our website and systems secure, including blocking spam; and
- meet our legal and professional obligations.
We do not sell your personal information, and we do not share it with anyone for their own marketing.
Health information
The reason for your visit and anything you write in our form may be health information, and we treat it confidentially. Once you become a patient, the information in your patient record is also protected by our Notice of Privacy Practices, which we give you when you become a patient and which explains your rights under HIPAA and Connecticut law. If this policy and that notice ever differ about your patient record, the Notice of Privacy Practices applies.
Our new-patient intake form is hosted by our practice-management provider, ChiroTouch, at intake.mychirotouch.com. Information you enter there becomes part of your patient record.
How information is shared
We share your information only with:
- Service providers that help us run our website and practice, such as website and database hosting, email delivery, website analytics (Google) and phone and text service. They may use your information only to provide their services to us.
- Legal and safety recipients, when the law requires it or when needed to protect someone's health or safety.
- A successor, if our practice is sold or merged, subject to this policy.
Text messaging: we do not share your mobile number or text-messaging consent with third parties or affiliates for their marketing or promotional purposes.
Supplement store and other websites
Our supplement store is run by Standard Process on its own website. When you order there, Standard Process's privacy policy and terms apply. The same goes for links to Facebook, Instagram, Google and ChiroTouch.
How long we keep information
We keep appointment requests, messages and related emails as long as we need them to respond to you and run the practice. Once you are a patient, we keep your records for as long as Connecticut law requires for healthcare records.
Security
We use reasonable safeguards, including encrypted connections (HTTPS) and access limited to staff who need it. No system is completely secure, so please do not send sensitive health details beyond what our form asks for. You can tell us more at your visit.
Your choices
- Texts: reply STOP to any text from us to stop receiving texts. See "SMS and text messaging" in our Terms and Conditions.
- Analytics: you can block cookies in your browser or use Google's opt-out add-on.
- Access, correction or deletion: you can ask what information we hold about you, or ask us to correct or delete it, by contacting us below. We may need to keep some records where the law requires it.
- Do Not Track: because we don't track visitors across other websites, our site does not change its behavior in response to browser "Do Not Track" signals.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them online. A parent or guardian should request an appointment on a child's behalf.
Changes to this policy
We may update this policy. When we do, we will post the new version here with a new effective date.
