Dear Practice Member,

In compliance with the Health Information Portability and Accountability Act (HIPAA) of 1996, Propper Chiropractic, LLC is required to conform with state and federal regulations in order to protect your (and your minor children’s) protected health information. HIPAA regulations protect all consumers and apply to all health care providers.

In complying with HIPAA regulations we have developed appropriate and necessary forms that must be signed and kept on file by each of our patients as well as our business associates. We will provide you with a copy of these forms.

We ask that you read the following forms thoroughly and return them to the office on your next visit. Parents, please note that we must have signed copies for each individual child.
